Greetings,

We are working on collecting information about how to best support faculty and staff caregivers during the upcoming academic year, with a special emphasis on providing youth programs in Fall 2020. 

To better assess the needs of faculty and staff, we've created this short survey. This survey can be filled out anonymously and shouldn't take more than 10 minutes to fill out. 

We encourage you to take the survey by this upcoming Wednesday, September 16th so that we can aim to provide services that will be accessible in Fall 2020.
